Generic application form
Letters, applications, and documents to be presented to the UAB can be formalised and delivered using this form.
Information about the procedure
- Follow the link and select the digital identification type you would like to use.
- Fill in your details, attach the specific form for the procedure chosen and any other supporting documentation.
- Validate and send.
- You will receive proof of application, which will be automatically registered.

Who can apply for the procedure

A physical party: the applicant is acting on their own behalf.

A physical party who is acting on behalf of a third party: any legal or physical party awards representation to a third party, using a digital certificate, so that they can undertake, on behalf of the party being represented, the UAB procedures and actions for which they have been authorised in the letter of authorisation
Note, the letter of authoritsation must be signed (by hand), scanned and attached to the application, if the type of applicant is a physical party representing a third party.
Deadlines
The Electronic Registry is operational 24 hours a day, every day of the year, but processing times are determined by the dates set out for each of the procedures.
To calculate deadlines, if applications or documents are received on a non-working day, they will be understood to have been delivered on the following working day.
The Electronic Registry is governed by official dates and times.
Supporting documentation
The documentation attached to the procedure:
- Must be generated without any viruses or any other malicious codes.
- Must be able to be visualised and printed without using a password.
- Must use one of the following formats: jpg, .jpeg, .odt, .odp, .ods, .docx, .xlsx, .pptx, .pdf, .png, .rtf, .svg, .tiff, .txt, .xsig, .xml
To improve the efficiency of the IT platforms and available communication channels, the size of attached documents is limited to a maximum of 5 MB, and a maximum of 5 documents may be attached.

- Register your application at any of the registry offices at the UAB in person.
